My fee for a 3-4 hour extended consultation is $400. This fee includes a brief initial screening phone call, review of your WHQ, ~2-2.5 hour clinical appointment and phone consultation with your referring medical or mental health professional. Time needed for an extensive chart review, extended phone calls/ emails or written reports will be charged at $200/ hr or $50/ 15 minute increments. My fee for phone consultation or briefer in-person appointments is $200/ hour, billed at $50/ 15 minute increments. Extra services such as written reports, extended phone calls, etc are billed at $200/hour or $50/ 15 minute increments. Payment is by check due at the time of service. I am not set up to take credit card payment. |
